About Our Family Health Team

The West Nipissing Family Health Team (WNFHT) was established in 2011 and is the largest primary care medical clinic in West Nipissing. The WNFHT is one of 27 family health teams across Northeastern Ontario working towards a vision that all Ontarians will have timely access to high-quality, comprehensive team-based primary care in their local community. The WNFHT serves approximately 65% of the population within West Nipissing. WNFHT comprises a group of interdisciplinary professionals working to improve the health and well-being of the patients served through prevention, identification and management of disease. In partnership with our affiliated physician group, the interdisciplinary team includes nurses, nurse practitioners, social worker, dietitian, pharmacist, occupational therapist, respiratory therapist, physiotherapist, psychiatrist and administrative staff.

Role of Providers

Family Physicians

Family physicians provide comprehensive health care to people of all ages. Family medicine is a form of primary care that provides continuing, comprehensive health care for the individual and family across all ages, sexes, diseases and parts of the human body. Our physicians deliver a range of acute, chronic and preventive medical care services that emphasizes health promotion and disease prevention. In addition to diagnosing and treating illness, our physicians also provide preventive care, including routine checkups, health-risk assessments, immunization and screening tests, and personalized counselling on maintaining a healthy lifestyle.

Our family physicians work collaboratively with other members of our health team to provide you with comprehensive care.

Nurse Practitioners

A Primary Care Nurse Practitioner (NP) is a Registered Nurse in the Extended Class with advanced university education specializing in Primary Care. We care for the physical, emotional, mental and social needs of our patients. As NPs working in a family health team, we work autonomously, as well as in collaboration with family physicians and other allied health professionals, to serve a diverse population of patients. One of the benefits of working as an NP in a family health team is having access to all the allied health professionals (social worker, pharmacist and dietician. And we work collaboratively with our family physicians to consult about a patient’s specific health need that may not happen to fall within our scope of practice.
